Reverendissime in Christo domine, domine praesul amplissime, post servitiorum meorum commendationem.

Licet ad Vestram Reverendissimam Dominationem, ut ad ceteros dominos consiliarios, maiestatis regiae domini mei clementissimi litteras excusatorias et intercessorias scripserim, attamen necessarium duxi ad Vestram Reverendissimam Dominationem pro hoc meo notario specialiter scriberem, ut is mearum precationum vigore penes Vestram Reverendissimam Dominationem subsidium sentiat g(ene)rosum or gr(ati)osumg(ene)rosumg(ene)rosum or gr(ati)osum. Precor itaque, quatenus Vestra Reverendissima Dominatio eum gratioso patrocinio iuvare non dedignetur, quod suum est, consequi possit, ut valeat, persuasum enim mihi habeo, quod eiusdem Vestra Reverendissima Dominatio tantum favoris, quantum ego eiusdem Vestrae Reverendissimae Dominationis famuli written over ooii written over os et amicis written over umumisis written over um subsidium impenderem, sit impensura, quod suo tempore promereri studebo et me in gratiam commendo Vestrae Reverendissimae Dominationis, quam Deus in longaeva sanitate optime conservet.
