Sigismundus Dei gratia rex Poloniae, magnus dux Litvaniae, Russiae, Prussiae ac Masoviae etc., dominus et heres

Reverende in Christo pater sincere nobis dilecte.

Grata est nobis admodum S(trenuitatis) or S(inceritatis)S(trenuitatis)S(trenuitatis) or S(inceritatis) Tuae sollicitudo, quam gerit in componendis subditorum illic nostrorum controversiis, e quibus nobis vehementer displicet, quae inter generosos Achacium Czema et Stanislaum Kostka intercessit, praesertim autem, quod una aliam generare videatur. Cum itaque priori finem imposuerimus et istius perfectam cognitionem non habuerimus, inaudita alia parte sententiam ferre non potuimus, sed ut certiores de ea reddamur, committimus S(trenuitati) or S(inceritati)S(trenuitati)S(trenuitati) or S(inceritati) Tuae cum aliis consiliariis nostris, ut examinato diligenter negotio nos de illo iuxta tenorem commissionis informet.
